Facilitator Paperwork Reminder Fall 2011 from Katja

Please complete items 1 to 3 within two weeks of the workshop.

1. Zone 4. All coaches register through zone4. Only online payment (credit card) is accepted this year. Please send me the registration form by e-mail as well as a hard copy by mail together with all the other paperwork.
2. You can send me your expense form electronically, but I will need your original with receipts by mail. I will only forward the expense form for you to get paid by CCO once I receive all the necessary paperwork.
3. Materials: Please get in touch with me early with regards to workbooks. I am shipping them by mail and need to have enough time (3-4 days) to get it to you. Having to send it last minute by courier is not necessary.
